At BoulderCentre for Orthopedics & Spine, the safety of student-athletes in the game and out in the world is our highest priority.
Available at our Boulder office, fellowship-trained sports medicine specialist Dr. Melanie Worley is offering sports physicals to athletes for the upcoming season.
Physicals are offered at a flat rate of $30. No insurance is needed and vaccinations are excluded.
Requirements:
- Please download, print, and complete the forms found on your child's school website and bring them to the physical
- An adult must be present
- Wear shorts and a T-shirt
- If your child wears glasses or contacts, they are required to wear them to the physical
